Which was a just retaliation to him, for having no bowels of compassion on his brethren whom he slew: until thy bowels fall out by reason of the sickness day by day; that continuing day after day, and even year after year, the space of two years, as appears from 2Ch 21:19.
This site's exposition
with a great plague will the Lord smite
on 2 Chronicles 21:14-15
Elijah's letter turns from indictment to sentence: a great plague on the people, the children, the wives and all the goods, and on Jehoram himself a disease of the bowels that will grow day by day until the bowels themselves fall out. The two clauses are not separate threats but one unfolding judgment, and the chapter will report both fulfilled in order, the household first and the king's own body last.

No marvel that hardened sinners are not frightened from sin and to repentance by the threatenings of misery in another world, which is future and out of sight, when the certain prospect of misery in this world, the sinking of their estates and the ruin of their healths, will not restrain them from vicious courses.
The Nineteenth Century2 voices
Until thy bowels fall out - This must have been occasioned by a violent inflammation: by the same death perished Antiochus Epiphanes, and Herod Agrippa.

hast made Judah and the inhabitants of Jerusalem . . . like to the whoredoms of the house of Ahab--that is, introduced the superstitions and vices of Phœnician idolatry (see on De 13:6-14). On this account, as well as for his unnatural cruelties, divine vengeance was denounced against him, which was soon after executed exactly as the prophet had foretold.
